Coordination of Support Services
Each person supported who receives services at ATCO, has a Support Coordinator who is responsible for coordinating all supports and services. The Support Coordinator ensures that the total spectrum of dreams, desires and needs for each person supported is addressed. These supports may include, but are not limited to, the following: coordination of assessments and teaching programs; medical, physical, psychological and emotional supports; finances; living arrangements; social activities and family contacts.
The Support Coordinator ensures that people's wants and dreams are solicited and known by all team members as well as ensuring that the necessary assessments and evaluations are completed. A comprehensive support plan is developed from this information and revised at least annually.
After the personal support plan is developed and the supports are put in place, the Support Coordinator monitors the services to ensure that supports are in place and the person's desires, wants, dreams and needs are being met. If at any time the person's desires, wants, dreams and needs are not being met or there needs to be a change in supports, the person's support team will meet to revise the plan.
